A clear referral pathway for families and professionals.

We welcome referrals from participants, families, support coordinators, allied-health professionals, schools, plan managers and other authorised representatives.

Support as a bridge to a wider life.One-to-one support can create a safe foundation for confidence, community and independence.
Who the service may suit

Participants who need more than a generic shift.

Motion & Meaning Support may suit boys and young men who benefit from a consistent worker, shared interests, purposeful activity and gradual pathways into community life.

  • approximately aged 8–30
  • self-managed or plan-managed funding for a suitable support
  • Northern NSW or Gold Coast location
  • goals relating to community participation, confidence, routine, social connection or independence
  • interest in active, creative, practical or community-based support
  • needs within current worker capability and service scope

Before referring

It helps to know:

  • the participant or authorised representative consents to contact
  • the broad support goals and preferred schedule
  • the participant’s interests and communication preferences
  • the funding-management type
  • any immediate factors affecting safe suitability

Please do not send an entire NDIS plan, detailed trauma history, medication list or sensitive incident records through the public form.

Referral process

Simple at the start. Thorough before service begins.

    Submit a referral

    Provide basic contact, location, funding, goals and interests.

    Initial review

    We consider service area, funding, availability and general scope.

  3. 3

    Introductory call

    Clarify the participant’s preferences, current supports and what a useful first meeting should involve.

    Complimentary Meet & Connect

    Meet the participant and authorised people, discuss fit and answer questions.

  5. 5

    Formal onboarding

    Only after both sides wish to proceed: service agreement, consent, safety information, pricing and practical arrangements.
